Please download and print out form #1 (all students) and form #2
(high school students only) and fill them out as described below.
Having the forms already filled out appropriately will save time
during the registration process.
Click on the title of the form you want to download: 1. Registration and Media Release Form: Please use one form per family. Complete the Parent/Guardian Information
and the name of each student (the teacherīs name and room number will be filled
in by the German Language School) and both parts of the Media Release form.
Please do not forget to provide all four signatures. Please bring the
completed Registration and Media Release Form with you to registration on Sept.
8, 2007.
High school students: Please take the Letter of Permission to your High
school Principal/ Principal Designate for his/her signature and have your
parents sign at the bottom of the page. Please bring the completed and signed
Letter of Permission with you to registration on Sept. 8, 2007 |
